Upload your COVID vaccine booster documentation – Feb. 12

While the definition of fully vaccinated continues to be either the two-dose series (Pfizer and Moderna) or the single-dose Johnson & Johnson vaccination, UK HealthCare is required to track all COVID vaccination information for the groups considered health care workers (see list of groups under vaccine mandate item).

Beginning Feb. 12, all faculty, staff, students and volunteers are required to upload any additional COVID vaccinations received (boosters) to the UK Health Corps Self-Report Tool.

If you have proof of full vaccination now, please ensure the appropriate documents are uploaded to the self-report tool as soon as possible. Remember to upload proof of any future boosters received as well.
